Perimenopause in the Sandwich Generation: When Life Feels Like a Panini Press
By Dr. Suzanne Tang, Inspire Natural Wellnesss
When Life Feels Like a Panini Press
The sandwich generation is a season of life marked by the demands of caring for aging parents while raising or supporting children, often while working full time, managing a household, and trying to preserve your own health.
With multiple high-pressure responsibilities squeezing us thin, it can feel less like a sandwich and more like a Panini press.
Add the erratic hormonal changes of perimenopause, brain fog, sleep disruption, anxiety, joint pain, exhaustion, body-composition changes, and metabolic shifts and that pressure can become overwhelming.
If you are navigating perimenopause while caring for everyone else, I want you to know: I am right there with you.
When Life Asks More of Us
This past year, my family faced several significant health challenges at once.
My father-in-law fell after a stroke and broke his hip and lumbar spine. He was sent home to our house needing extensive support, including diapers, a catheter, and a walker. Less than a week later, on Christmas Eve, we took him to the emergency room, where he was diagnosed with sepsis.
When he returned home, catheter- and diaper-free, he continued to face numerous health complications that we helped him navigate. With faith, patience, prayer, consistent care, and good nutrition, he made an incredible recovery. He has recently moved back to his own home and is living independently again.
While we were managing my father-in-law’s care, my father was diagnosed with Parkinson’s disease. The diagnosis has been difficult for him physically and emotionally.
At the same time, my son sustained multiple traumatic brain injuries during his senior year of high school while applying to college. The injuries significantly affected his mental, emotional, and physical health. After months of therapy and functional medical support, I am deeply grateful to see him progressing in his recovery. We recently moved him into his dorm at UC Riverside, and he is adjusting to college life.
Even when we are accustomed to caring for others, there are seasons when life asks us to carry more than we ever imagined possible.
Perimenopause Symptoms in the Sandwich Generation
I have supported women through their perimenopausal journey for more than two decades. Yet walking through my own hormonal transition while my family faced these significant demands gave me a much deeper level of empathy and understanding.
During this period, my brain fog and forgetfulness became debilitating. My sleep was disrupted, anxiety heightened, and my joints ached. My blood sugar and cholesterol were rising. Negativity felt pervasive. Belly fat clung to me, and exhaustion was constant.
These are not minor inconveniences. They are common, real experiences during perimenopause and they can substantially affect quality of life, relationships, work, confidence, and long-term health.
For many women in the sandwich generation, the challenge is not simply managing perimenopause symptoms. It is managing those symptoms while everyone else still needs something from you.
Why Normal Hormone Labs Do Not Tell the Whole Story
Perimenopause is often misunderstood as a simple, steady decline in hormones. In reality, it is commonly characterized by erratic and significant hormonal fluctuations throughout the menstrual cycle.
Those fluctuations can create intense symptoms even when a single lab measurement of estradiol or progesterone appears to be in a healthy range. This is why lab results alone do not determine when a woman needs support.
At Inspire Natural Wellness, we look at the full clinical picture: symptoms, health history, goals, risk factors, lifestyle, sleep, stress load, metabolic health, and the demands of the season a woman is living in.
Waiting only for hormone levels to decline can leave women struggling unnecessarily through years of symptoms. As female hormones begin to decline, the risk of cardiovascular disease, type 2 diabetes, dementia and Alzheimer’s disease, osteoporosis, osteoarthritis, depression, anxiety, and certain cancers increases significantly.
Supporting women through perimenopause is not about dismissing symptoms as an inevitable part of aging. It is about taking women’s health seriously, understanding the larger clinical picture, and acting with intention.
My Experience With Hormone Replacement Therapy
To keep up with the physical and emotional demands of this season, I began hormone replacement therapy.
My treatment included estradiol, estriol, and testosterone cream, along with oral micronized progesterone. I also continued DHEA supplementation.
Hormone support was one meaningful part of a broader wellness foundation already in place. I continued to focus on nutrition, movement, sleep hygiene, mindfulness, nervous-system regulation, herbal and nutritional supplementation, and homeopathic recommendations based on my bio-meridian scan. I was also supported by our very own Jill Brew.
After three months of HRT, alongside the wellness pillars already in place, I felt like myself again.
I could complete my train of thought. I woke up more rested. My energy was steadier throughout the day. I managed stress more easily, felt stronger and leaner, and saw meaningful improvements in my blood sugar and cholesterol levels.
We see this transformation with our members every day. Walking through it myself gave me renewed perspective and deep respect for every person who chooses to put their health first when life is difficult, demanding, and uncertain.
It is not always easy to do the work. But it is deeply worth it.
Because you are worth it.

You Do Not Have to Do It Alone
Moving through this period with emotional and spiritual groundedness was not easy. It required faith, surrender, prayer, positive mindset work, and a willingness to ask for and receive help.
My husband and I worked together as a team. We reached out to resources, took breaks when we could, and supported one another and our individual needs through very difficult moments.
As I look at the nest after launching our last chick and see my father-in-law back home and doing well, I have more vitality and capacity to return to myself. I have started asking: What will bring me greater joy and deeper connection in this next chapter, even while I am still in the Panini press?
For me, that has included starting choir class at Saddleback College and joining a small group at church with my husband. Both have been incredibly soulful and nourishing.

Frequently Asked Questions About Perimenopause
Can perimenopause symptoms occur when hormone labs appear normal?
Yes. Perimenopause can involve substantial hormone fluctuations throughout the menstrual cycle. A single lab result may not reflect the hormonal variability that contributes to symptoms such as sleep disruption, brain fog, anxiety, fatigue, joint pain, and metabolic changes.
What are common symptoms of perimenopause?
Common symptoms may include irregular cycles, hot flashes, night sweats, sleep disruption, brain fog, mood changes, anxiety, fatigue, joint discomfort, body-composition changes, and shifts in blood sugar or cholesterol.
When should I seek care for perimenopause?
Seek care when symptoms are affecting your quality of life, sleep, work, relationships, mood, energy, or physical health. You do not need to wait for symptoms to become severe before pursuing individualized support. However, if you are not experiencing symptoms and you are nearing menopause, which is marked by a year without a menstrual period, still seek care to help prevent diseases associated with menopause, such as heart disease, osteoporosis, Type 2 Diabetes, dementia and Alzheimer’s disease.
Can hormone replacement therapy help during perimenopause?
Hormone replacement therapy can be an important component of a comprehensive perimenopause plan for women who are appropriate candidates. At Inspire Natural Wellness, recommendations are individualized based on symptoms, health history, treatment goals, and clinical findings.
